We report three cases with high serum amylase levels due to different causes among out-clinic patients with type 1 diabetes mellitus. Case 1 is a 14 years old boy. He was diagnosed as having diabetic ketoacidosis, when his serum amylase was 1.285IU/l (92% of serum amylase was salivary type). Case 2 is a 17 years old boy. He had nausea and abdominal pain, and his serum amylase was 651IU/l. Abdominal echo and CT examinations revealed pancreatitis. Case 3 is a 15 years old boy. His serum amylase was 322IU/l. He did not have pancreatitis, and macroamylasemia was diagnosed by electrophoresis examination and low clearance of urinary amylase. Hyperamylasemia in type 1 diabetes mellitus is induced by different mechanisms.
